Referring Dentists
Thank you for referring to our office. The patient care is our number one priority, this is why we schedule and perform consultations prior to appointing patients for treatment. Patient Info

First Visit
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. A complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip and any X-rays if applicable.
  • A list of medications you are presently taking.
  • If you have dental insurance, please bring a card with you, we will scan it into your chart. Though, we are an out of network office we will submit to your insurance electronically as a courtesy, and should they reimburse, it will go directly to the patient completely by-passing our office.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental procedures (i.e antibiotics, for pre-med.)

X-Rays
If your previous dentist has taken recent x-rays (within 1 month), you may request that they forward them to our office. However, it is necessary that we take our own images, sometimes at a different angle and/or in adjacent areas to reach our final diagnosis.

We provide our patients with the latest technology in digital radiography.  All radiographs are taken with Dexis digital sensors, providing instant viewing on our computer while significantly reducing radiation levels compared to film-based x-rays.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you.
